Borderline:

Gleason score 4+3=7. This small focus of tumor shows predominantly pattern 4 with poorly-formed glands. However, isolated individual well-formed glands on a small sampling should be noted, as undoubtedly the entire prostate would show more pattern 3. If this were extensive tumor with pattern 4 occupying 99% of the case on biopsy and only rare, isolated individual glands, the tumor would be best classified as Gleason score 4+4=8.
